Covenant Care, headquartered in Aliso Viejo, CA, operates over 50 healthcare and rehabilitation centers in 7 states across the U.S.
Covenant Care employs over 8300 health care professionals throughout the network of facilities. Facilities offer 24-hour-a-day care with therapy and skilled nursing services tailored for both short-term and long-term care. A team of facility employees consists of an Executive Director, Director of Nursing, MDS Coordinator, Admissions Director, Rehab Director, Dietary Supervisor, Activities Director, Social Services Director, Office Managers, CNAs, RNs, and Therapists.

Post-Acute / Skilled Nursing and Rehabilitation Centers
Post-Acute and Skilled Nursing Facilities (SNFs) both provide the same services with increased emphasis on rehabilitative therapies for convalescent patients. Covenant Care facilities offer both skilled short-term and long-term care under the direction of our Medical Directors and treating physicians.
“In terms of rehabilitation in skilled nursing facilities, hospitals make the arrangements for follow-up patient care after an acute hospital stay, like after a surgery. When released from the hospital, a patient transfers to the skilled nursing home to receive hands on care from nurses.
